BROKER-DEALER OR INVESTMENT ADVISER COMPLAINT
The Division of Securities ("Division") is aware of your concerns regarding a
licensed Broker-Dealer, Agent, or Investment Adviser. If you wish to file a complaint
against an individual for violations of the securities laws, you must first complete a
complaint form (available from the Division). Please attach any documents (i.e.
photocopies of account statements, transaction confirmations, any correspondence or notes
from conversations) that will help us better understand your complaint. If you have any

We know this matter is very important to you and that your complaint has been made in
good faith. Disputes often arise in the sale of securities as a result of poor
communication. Often these disputes can be resolved by frank and early discussion between
you and the Investment Adviser's or Broker-Dealer's branch manager. In addition to filing
a complaint with the Division, it is suggested that you contact the firm's branch manager
to attempt to resolve the problem.
Following a review of your complaint, your case will either be retained for further
investigation or referred to another agency for action. If further investigation is
warranted, a member of our investigative staff will be assigned to the case who will
contact you. However, if you receive additional information important to an understanding
of your complaint, you should provide it to the Division as soon as possible.
This agency is responsible for the administration and enforcement of the Utah Uniform
Securities Act ("Act"). The Division is not authorized to bring a civil action
on your behalf or recover any funds which may have been lost. Furthermore, the Division is
prohibited from acting as your attorney or providing legal advice. Any person who has lost
money as the result of an unlawful securities transaction should consider contacting a
private attorney about the possibility of arbitration or a lawsuit to recover lost funds.
These rights are limited in time, and any action by the Utah Division of Securities will
not toll the statute of limitations. If you have not already done so, you should contact
your private attorney immediately to determine your rights.
